Patient Information

Pricing, Insurance, and FAQs

  • We have several ways to schedule an appointment to make it easy for everyone’s schedule:

    1. Book online using the “Book Online” button at the top of this page

    2. Fill out the form on our contact page and someone will get back to you in 1-2 business days

    3. Call or text (267) 223-4366

If the issue is emergent (such as pain or late pregnancy issues), please don’t hesitate to call/text us directly at (267) 223-4366!

  • As much as we all wish we could give you a definitive answer, the truth is that it depends on so many factors. We typically see initial improvement within 2-3 sessions, and our average number of patient visits is 5-7.

  • We are out-of-network providers with insurance, meaning that we choose to not contract with any insurances. This allows us to provide a much higher quality, overall more holistic care, trauma-informed therapy, and various payment options to those who need them.

Due to being out-of-network, we are unfortunately not able to see anybody who is currently on Medicare.

  • We are able to easily provide you with superbills. In submitting a superbill, you may or may not be reimbursed depending on your specific insurance plan. Some plans reimburse 20% of the session, and some will reimburse up to 80%. We are unable to tell you how much you’ll be reimbursed.

We do partner with Reimbursify so that they can do all of it for you - and the best part is that the first superbill to submit is FREE using them!

If your insurance would like to know the billing (CPT) codes that we use, you’ll find them below:

For an evaluation, we always use one of these:

97166: Occupational Therapy Evaluation with Moderate Complexity (for our OTs)

97162: Physical Therapy Evaluation with Moderate Complexity (for our PT)

In addition to 1 evaluation code and during future treatment sessions, we will also use a combination of these treatment codes:

97110: Therapeutic procedure; therapeutic exercises to develop strength and endurance, range of motion and flexibility

97112: Neuromuscular re-education of movement, balance, coordination, kinesthetic sense, posture, and/or proprioception for sitting and/or standing activities

97140: Manual therapy techniques, 1 or more regions, each 15 minutes

97530: Therapeutic Activities with direct, one-on-one contact with the patient using dynamic activities to improve functional performance

97535: Self-Care/Home Management Training (e.g., activities of daily living (ADL) and compensatory training, meal preparation, safety procedures, and instructions in use of assistive technology devices/adaptive equipment)

In addition to these, for pediatrics & infant development we can also use:

97533: Sensory integrative techniques to enhance sensory processing and promote adaptive responses to environmental demands

92526: Treatment of swallowing dysfunction and/or oral function for feeding

  • Please contact us if you’re finding that finances are a barrier to working with us! Ways that we try to help remove financial barriers include:

    • Payment Plans available

    • We do have a small sliding scale fund to help patients who are in financial need

Give us a call or fill out our contact page to chat more about options!

  • An initial evaluation will begin with a lengthy chat about your medical & social history, your goals, and your daily activities & routines.

Depending on the reason for evaluation, we will usually move into a functional movement assessment to see how your joints & biomechanics could be reflecting the issues you’re having (or to improve upon if we’re working in a preventative model).

Following our external assessment, we can do an internal assessment if indicated and only with your explicit consent. We tell you every single step of what we’re doing with the help of our pelvic model, and help you connect to your pelvic floor!

At the end of the evaluation, we’ll go back to talking about your goals & come up with a plan of care to get you to those goals!

  • You may not believe this, but trust us. While pelvic floor therapy is beneficial for everyone, it’s most helpful for those with bladder, bowel, and sexual health issues or concerns.

If you don’t have any concerns with these, you’re still welcome to come in for an evaluation and to learn about how to use your core and pelvic floor optimally!

Questions about services, insurance, and more?

Pricing

Flourish Pelvic Health & Wellness is out-of-network with all health insurance companies. This allows us to truly provide the highest quality of care, without being limited to a diagnosis code or therapy caps. We often also save patients money this way because with a higher quality of therapy, fewer sessions are often needed!

We believe in transparency with our pricing, so we list our prices for you to see before giving us a call or booking online! 10% of our full-price sessions go into our sliding scale fund to make sure finances are never an issue.

The best part about being out-of-network with insurance is that this also allows us to offer package prices, memberships, and sliding scale pricing to those in need. If you’re interested in sliding scale, please reach out to us directly at admin@flourishpelvichealth.com.
